Optimal Medical Group was founded on a simple but powerful conviction: that people in Fresno and the Central Valley deserved access to the same caliber of precision, personalized medicine available at the country's most elite institutions, delivered with warmth, attentiveness, and genuine care.
That conviction belonged to Dr. Juan Chavez, MD and Dr. Lucia Mireles-Chavez, MD, physician partners in medicine and in life, who built OMG from the ground up with an uncompromising commitment to their patients and their community.

A Practice Unlike Any Other
From the beginning, the Chavezes refused to practice medicine the conventional way. Dr. Juan brought rare, multi-disciplinary expertise spanning internal medicine, critical care, lipidology, obesity medicine, anti-aging and regenerative medicine, hyperbaric medicine, men's health, and exercise physiology, consistently ranking in the top 10th percentile nationally.
Dr. Lucia brought equally deep specialization in women's health, pediatric wellness, aesthetics, and functional medicine, creating a practice that could genuinely care for the whole family across every stage of life.
Together, they built something Fresno had never seen: a practice where board-certified physicians actually listened, where treatment plans were truly customized, and where patients weren't rushed, they were partnered with. The Search for a Successor
As the years passed, Dr. Juan and Dr. Lucia began thinking carefully about the future, not just their own, but the future of every patient who had trusted them. They knew that when the time came to step back, nothing less than a physician of extraordinary caliber would do.
For nearly two years, they searched nationally. They evaluated physicians not only on credentials, but on character, on how they listened, how they communicated, on whether they shared the deeply human approach to medicine that had defined OMG from day one.
That search led them to Dr. Muhammad J. Anwar, MD, MHA.

In mid-2026, after working alongside Dr. Anwar and witnessing firsthand how he engaged with patients, with precision, empathy, and genuine attention, the Chavezes made the decision to advance their retirement timeline and pass the practice to him with their fullest confidence and endorsement.
They didn't simply hand over the keys. They wrote personal letters to every patient. They hosted an open house. They made personal video introductions. Every step of the transition reflected the same care and intentionality with which they had always practiced medicine.
It was, as Dr. Juan put it: "The most important decision Lucia and I have made for this practice, and we made it with our patients in mind."
The Standard Continues
Dr. Muhammad J. Anwar, MD, MHA now leads Optimal Medical Group as its physician-owner, carrying forward the legacy the Chavezes spent years building, and taking it further.
Triple board-certified in Internal Medicine, Obesity Medicine, and Anti-Aging & Regenerative Medicine, Dr. Anwar brings a rare fusion of elite performance medicine, academic medical leadership, and deeply personalized concierge care. As former Concierge Physician for New York City Football Club (NYCFC) and Inaugural Medical Director of Student Health at Albert Einstein College of Medicine, he has operated at the highest levels of medicine, and now brings that expertise to every patient in Fresno.
His specialties, longevity medicine, hormone optimization, metabolic health, GLP-1-based weight management, and precision primary care, represent the leading edge of where medicine is headed. But what distinguishes Dr. Anwar above all else is the same thing that distinguished his predecessors: he treats every patient as a unique individual, not a condition to be managed.
